AUTHORIZATION AND RELEASE
This constitutes my consent and authorization to the disclosure and delivery of any information or material in the records of former employer(s), listed here,
to Magee Construction concerning my character, qualifications, and employment history for use by Magee Construction in determining my suitability for employment with Magee Construction. This authorization is executed with full knowledge and understanding that Magee Construction will take measures to protect the aforementioned information against unauthorized disclosure to any parties not having a legitimate need for it in the discharge of official business.
I hereby RELEASE and HOLD HARMLESS the aforementioned Magee Construction and my former employer(s) (listed above), their respective directors, employees, agents, and representatives from any and all claim of liability for damages resulting from the disclosure and delivery and use of such information and material as contemplated by this Authorization and Release and resulting from a decision by Magee Construction not to employ me as the result of compliance or any attempts at compliance with this Authorization, except for any damages resulting from knowingly disclosing or delivering false or misleading information about me,
A copy of this Authorization, Release, and Hold Harmless shall be as effective and valid as the original. This Authorization and Release shall be valid for six months from the date indicated below.
PRE-EMPLOYMENT STATEMENT

I understand and agree that:
1. The information that I have provided on this Application is true and complete to the
best of my knowledge. Any misrepresentation or omission of any fact in my application, resume, or any other materials, or during interviews, can be justification of refusal of employment, or if employed termination from Magee Construction Company's employ,
2. Any offer of employment I may receive from Magee Construction Company is contingent upon my successful completion of the company's total pre-employment screening process, including the company's receiving references that it considers satisfactory, and my satisfactory completion of any post-offer pre-employment medical examination that the company may require. I also agree, if employed, to submit to a medical examination at any time at the company's request. I hereby consent to having the results of any post-offer pre-employment or post-employment medical exams I may be required to take disclosed to Magee Construction Company.
3. I understand that as a condition of employment, I may be required to undergo and successfully pass a screening for alcohol and/or drug. I also understand and agree that, if employed, I may be required to submit to an alcohol or drug screening under certain circumstances as outlined in the Company's policy. I herby consent to having the results of any such alcohol or drug screening I amy be required to undergo disclosed to Magee Construction Company.
4. I authorize and request that all of my present and former employers and those individuals I have listed as personal references furnish information about my employment record, including statement of the reason for the termination of my employment, work performance, abilities, and other qualities pertinent to my qualifications for employment, herby releasing them from any and liability for damages arising from furnishing the requested information
5. I authorize Magee Construction Company to conduct a criminal history background check and an investigation of my driving record. This investigation will be conducted after an offer of employment and prior to actual employment with Magee Construction Company if I am offered a position.
6. I agree to comply with the policies, rules, regulations, and procedures of the company and understand that my employment and compensation can be terminated with or without cause or notice, at anytime, at the option of either the company or myself. I further understand that no manager or representative of the company, other than the President, Wayne Magee, has any authority to enter into any agreement with me for employment for any specified period of time or to make any agreement, if made, shall not be enforceable unless it is in writing and signed by me and Wayne Magee. I also understand and agree that no employee or representative of the Magee Construction Company has the authority to make any verbal promises, commitments, or statements of any kind at any time that are legally binding on the Company.
